• Photocopies of the identities of the parties involved.

Address and Income:

• Statement of the home address and profession of the parties involved.
• AFM and income tax office of the parties involved.
• Social Security Number (AMKA) of the parties involved.

Nationality:

• In the event that a party is a citizen of an EU member state, a photocopy of a valid passport is required.
• In the event that a party is a third-country national (non-EU), a photocopy of a valid passport and the following are required:
  • Entry visa (Visa) of any type.
  • Valid residence permit.
